Fill in every box

Click each box and type your answer. The form is 1 page. Make sure you fill in:

• Your name, title and your organization's name
• Federal Tax ID number, and check Yes or No for tax exempt
• Accounts Payable name, email, phone, fax and address
• Type of entity: check Government, Public School, College or University. If you are a government agency, also write Federal, State or Local next to it.
• Printed name and email at the bottom

Sign the form

The form must be signed. Pick whichever is easier:

• Sign on screen: in Adobe Acrobat Reader, click Fill & Sign → Sign, then type or draw your signature.
• Sign on paper: print the filled-out form, sign it with a pen, then scan it or take a clear photo of every page.

Save the file

Click File → Save (or press Ctrl + S on Windows, Cmd + S on Mac). Remember where you saved it.

Email it to us

Send an email to [email protected] with:

• The signed, filled-out form
• Your tax-exempt certificate (if tax exempt)
