Fill in pages 2 and 3

Page 1 is just instructions. Click each box on pages 2 and 3 and type your answer:

• Business contact information: legal name, DBA, purchaser, owners, address, phone, Accounts Payable contact, Tax ID, start date, and check your business type
• Bank reference: your bank's name, address and bank contact
• Trade references: 3 suppliers you buy from on credit
• Page 3: title, printed name and date

Sign the form

The form must be signed. Pick whichever is easier:

• Sign on screen: in Adobe Acrobat Reader, click Fill & Sign → Sign, then type or draw your signature.
• Sign on paper: print the filled-out form, sign it with a pen, then scan it or take a clear photo of every page.

Page 3 has two signature spots. The second one must be signed by your President, VP or CFO.

Save the file

Click File → Save (or press Ctrl + S on Windows, Cmd + S on Mac). Remember where you saved it.

Email it to us

Send an email to [email protected] with:

• The signed, filled-out form (all 3 pages)
• A copy of your business license or resale certificate
